Health History and Emergency Care Plan

Every child. Lists health conditions such as asthma, diabetes, epilepsy, or food allergies, with a care plan for each.

Every child under 5, filled out by your child's doctor. Under 2: an exam from up to 6 months before to 3 months after starting, then every 6 months. Ages 2–5: from up to 1 year before to 3 months after starting, then every 2 years.
